Transition
Care Places

Hall & Prior operate Transition Care Places (TCP) in both a residential and community setting, in partnership with the Western Australian Department of Health.

Transition Care is a specialised rehabilitation and re-enablement focused program for older people who have had a period of care in hospital. Patients are referred from hospital discharge coordinators and receive goal-focused care for up to 12 weeks, aimed at improving participants’ physical and cognitive functions.

TCP participants have increased access to Occupational Therapists, Physiotherapists, Speech Pathologists and Dieticians, in addition to having any personal care or nursing needs met by our team of Personal Care Workers and Registered Nurses. There is also specialist input available from a Geriatrician if required.
TCP is available in a residential aged care home at our services in:
  • Belmont
  • Carlisle
  • East Fremantle
  • Harvey
  • High Wycombe

TCP is available in the community (in your own home) in:

  • The Perth Metropolitan Region
  • Harvey and surrounds

Accessibility

Dual Funding

They also accommodate residents who are jointly funded by the NDIS and Aged Care.

Interim Care

Provided to individuals who need support and care after a hospital discharge to help them regain independence before potentially returning home or moving into permanent residential care.

Respite Care

Offers short-term care to give primary caregivers a break while ensuring the individual receives necessary support and supervision.

Support for Recovery

Hall & Prior’s homes provide clinical, therapeutic, personal care, and community support to help residents recover and adjust during this transitional period.
