Hall & Prior operate Transition Care Places (TCP) in both a residential and community setting, in partnership with the Western Australian Department of Health.

Care Places
Transition Care is a specialised rehabilitation and re-enablement focused program for older people who have had a period of care in hospital. Patients are referred from hospital discharge coordinators and receive goal-focused care for up to 12 weeks, aimed at improving participants’ physical and cognitive functions.
TCP participants have increased access to Occupational Therapists, Physiotherapists, Speech Pathologists and Dieticians, in addition to having any personal care or nursing needs met by our team of Personal Care Workers and Registered Nurses. There is also specialist input available from a Geriatrician if required.


TCP is available in a residential aged care home at our services in:
- Belmont
- Carlisle
- East Fremantle
- Harvey
- High Wycombe
TCP is available in the community (in your own home) in:
- The Perth Metropolitan Region
- Harvey and surrounds
