Hall & Prior are a NDIS Registered Provider, we are approved by the NDIS Quality and Safeguards Commission to deliver NDIS funded supports and services.

Under our Fresh Fields NDIS Registration we meet specific quality and safety standards outlined in the NDIS Practice Standards and adhere to the NDIS Code of Conduct.

As a Registered Provider we can support your endless potential of possibilities, through your agency, plan, or self-managed NDIS Plan.

Dual Funded NDIA/Aged Care Residents

Hall & Prior provide accommodation, clinical supports and care in their Residential Aged Care homes, to a number of residents who have dual funding through the Aged Care and NDIA funding stream.

The NDIS Possibilities Disability Support team provide two dedicated Liaison and Compliance Officers. They both play a crucial role in building strong rapport with our NDIS participants living in our Residential Aged Care Homes (RACH).

They develop working relationships with each participants NDIS Stakeholder Team and support our Residential Aged Care staff to navigate NDIS processes and understand how to assist participants to get the most out of their NDIS plan.

Importantly our Liaison and Compliance Officers, also ensure our reporting compliance as a registered implementing provider with the NDIS Quality and Safeguards Commission.

From Hospital to Home (FH2H)

Hall & Prior and the WA Department of Health (DOH) have partnered to support the safe discharge of medically fit patients who no longer need hospital care and are awaiting transition to person-centred independent living within their community.

Our FH2H program, located in South Perth, helps our participants to understand the support they need, our Team work with an individual to create goals and plan the steps for a successful transition back into a safe community minded accommodation and environment.

During the 12-18 week transition program our FH2H Team provides an individual with support services in interim accommodation, which meets the needs of people with their related disability, mental health and/or social needs.

The FH2H Team arrange longer term accommodation, community, capacity building and or behaviour management supports via funding from the National Disability Insurance Scheme (NDIS) or another program.

Referrals for our FH2H Program are made via a Perth metropolitan public hospital, social worker to the DOH, Long Stay Working Group.

Supported Independent Living

Our NDIS Possibilities Disability Support team provide SIL services to individuals who reside in our SIL homes currently in two locations, Manning and Bayswater.

We support you in your home, developing your skills and confidence to live more independently, with a plan dedicated to your unique needs and goals.

Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA)

The NDIS Possibilities Disability Support team have recently partnered with Nesti Housing to provide Supported Independent Living (SIL) supports in their Specialist Disability Accommodation homes in Beckenham.

We have vacancies available for individuals who have SDA and SIL funding in their NDIS Plan at Nesti Housings brand new SDA homes in Peckenham.

Community Participation Supports

Our NDIS Possibilities Disability Support team, have a dedicated team who support people to engage in community activities that will enhance social connections, promote social inclusion and develop community engagement skills.

Our goal is to foster a sense of belonging in your community, increase your independence and improve your health and wellbeing.

Support Coordination Level 2

Our NDIS Possibilities Disability Support team has a dedicated Support Coordinator to champion the understanding and implementation of a participants NDIS plan.

Our Support Coordinator will walk with you, focusing on building your capacity throughout your NDIS journey, establish connections with NDIS providers, all with an aim for you to navigate the NDIS independently.

Short Term & Medium Term Accommodation (STA & MTA)

Our world-class Karingal Green Health & Aged Care Community in High Wycombe offers dedicated beds and services and provides age and lifestyle appropriate programmes and facilities for young people.

Our Allied Health Team can support your capacity building needs during your STA and or MTA stay with us. 

We have vacancies within our homes for STA or MTA stays for NDIS participants. These vacancies would suit participants that require access to 24-hour clinical and personal care. 

Karingal Green is a space for healing with access to state-of-the-art facilities including a wellness centre, that houses a hydrotherapy pool, gymnasium, hairdresser, café, and allied health clinic.
