More information about home care packages

A home care packages is a Commonwealth program involving individually planned packages of care designed to enable you (or your loved one) to continue living as independently as possible in your own home.

Levels of care

All home care packages offer the same range of care and services: the main difference between the different levels is the amount of care and services that you or your loved one will receive.

  • Level 1 and 2 Home Care Packages – support for people with low-level care needs
  • Level 3 and 4 Home Care Packages  – support for people with high-level care needs.

Services available

Examples of care and services that may be provided as part of a home care package include:

  • personal care – assistance with showering or bathing, dressing and mobility
  • support services – assistance with housework, laundry, shopping, transport, and social outings
  • clinical care  – nursing and other health support such as physiotherapy and occupational therapy.

Eligibility

To access a home care package, you will need to be assessed by a member of an Aged Care Assessment Team (ACAT). The Australian Government website My Aged Care has a list of all the ACATs.

Consumer directed care

If you choose White Oak or Clarence Estate Home Care as your preferred home care provider, we will meet with you and help plan your care program based on your specific needs and preferences.

While consumer directed care is an essential part of how we offer home care packages as a Government requirement, it is also a core philosophy for how we provide services to all our clients on all our programs.

We will assist you to get the most value from any program that you are on. The choice of the services that you receive is up to you within the guidelines of your program, and we will assist you to make choices that will suit you and your preferences.
